Question on how and what requirements needed for US Pass Port?

Hello all.

I have a couple of question's pertaining on how to obtain a US Pass Port.

I have to obtain my Pass Port/Pass Card for my newly job which I will have to travel to Canada

1. When filling out a Pass Port app under parent's birth date's, for example I know the month and year my father was born and place of birth but I do not know my mothers only thing I I know about my mother would be her name, place of birth, and also the year. What would I do in this situation?

2. Under travel plans since I am not going out of country until I start my new job would I have to put unknown? for example date of trip, length of trip, and countries to be visited.

3. Under have you ever been married, since I never been married before do I just fill out the boxes by putting N/A?

Basically I would like to know when question's are asked on the app, and there is no answer for those question's, what should I put? Also under the year's do I leave the year's blank or put N/A or Unknown.

A: Write the closest date you can think of.

2. Under travel plans since I am not going out of country until I start my new job would I have to put unknown? for example date of trip, length of trip, and countries to be visited.

A: Write under that section your plans. i.e. you're going to Canada on 'x' date, and will be staying there for 'x' time.

3. Under have you ever been married, since I never been married before do I just fill out the boxes by putting N/A?

Well I don't know when I will be going to Canada since I haven't been hired yet. The company that will probably hire me has to send me to orientation first to get approved for the job, before I go to orientation I have to show proof I have a passport so How can I put a date if I wouldn't know if i will get hired yet?

Ask the company about the date for the job orientation day, and write that on the passport application form. Also, have you tried asking the US embassy about you not having a passport and what to do?
